鼻肠管与鼻胃管在重型创伤性脑损伤病人肠内营养安全性比较的Meta分析

目的 采用Meta分析评价鼻肠管(nasaljejunal tube,NJT)与鼻胃管(nasogastric tube,NGT)在重型创伤性脑损伤(traumatic brain injury,TBI)病人肠内营养的安全性比较。方法 检索1950年1月-2023年8月Medline、EMBase和Cochran图书馆、中国学术期刊全文数据库、万方数据库等,筛选有关重型TBI病人应用NJT和NGT肠内营养的研究文献。应用Review manager 5.4.1进行Meta分析。结果 纳入30个研究,总计2952例病人。Meta分析显示:(1)NJT组与NGT组的呕吐反流率OR合并值为0.21,95%可信区间(Confidence Interval,CI)为(0.16,0.28),两组差异有统计学意义(P<0.01)。(2)NJT组与NGT组的误吸率OR合并值为0.23,95%CI为(0.16,0.34),两组差异有统计学意义(P<0.01)。(3)NJT组与NGT组的腹泻率OR合并值为0.66,95%CI为(0.52,0.84),两组差异有统计学意义(P<0.01)。(4)NJT组与NGT组的消化道出血率OR合并值为0.65,95%CI为(0.48,0.86),两组差异有统计学意义(P<0.01)。(5)NJT组与NGT组的肺部感染率OR合并值为0.30,95%CI为(0.23,0.40),两组差异有统计学意义(P<0.01)。结论 与NGT相比,NJT在重型TBI肠内营养时,安全性更高。

Abstract:

Objective To evaluate and compare the safety of nasojejunal tube(NJT) and nasogastric tube(NGT) for enteral nutrition in patients with severe traumatic brain injury by a meta-analysis. Methods A literature search was conducted from January 1950 to August 2023 in databases including Medline, EMBase, Cochrane Library, China National Knowledge Infrastructure(CNKI), and Wanfang Database. Studies related to the application of NJT and NGT for enteral nutrition in patients with severe traumatic brain injury were screened and selected. Meta-analysis was performed using Review Manager 5.4.1. Results A total of 30 studies involving 2952 patients were included. Meta-analysis showed that:(1) The pooled odds ratio(p OR) of vomiting and reflux in the NJT group compared to the NGT group was 0.21, with a 95% confidence interval(CI) of(0.16, 0.28), and the difference between the two groups was statistically significant(P<0.01).(2) The p OR of aspiration rate in the NJT group compared to the NGT group was0.23, with a 95% CI of(0.16, 0.34), and the difference between the two groups was statistically significant(P<0.01).(3) The p OR of diarrhea rate in the NJT group compared to the NGT group was 0.66, with a 95% CI of(0.52, 0.84), and the difference between the two groups was statistically significant(P<0.01).(4) The p OR of gastrointestinal bleeding rate in the NJT group compared to the NGT group was 0.65, with a 95% CI of(0.48, 0.86), and the difference between the two groups was statistically significant(P<0.01).(5) The p OR of pulmonary infection rate in the NJT group compared to the NGT group was 0.30, with a 95% CI of(0.23,0.40), and the difference between the two groups was statistically significant(P<0.01). Conclusion Compared to NGT, NJT exhibit higher safety during enteral nutrition in patients with severe traumatic brain injury.
